In December 2023, a young person arrived at Enidaso House, part of Act Care’s residential support services, following a period of profound instability. Diagnosed with Autism Spectrum Disorder, Learning Disability, Attachment Disorder, and pseudo-psychotic symptoms within the context of autism, he had endured early life abuse, school exclusion, substance misuse, and the bereavement of his adoptive mother.
Arrival and Identity
Born biologically female, he was undergoing gender transition to male and received full support from therapists, medical professionals, care staff, and family. He arrived via secure transport with a 4:1 staffing ratio, deeply mistrustful, disconnected, and fearful of forming attachments.

Initial Challenges
His lifestyle was marked by poor nutrition, surviving on pot noodles and sweets, neglected hygiene, and high-risk behaviours including property damage, theft, and absconding. He weighed 80.7 kg and was disengaged from both staff and family, refusing to learn names or build relationships.

The Act Care Approach
The Enidaso team responded with a trauma-informed, person-centred strategy:
- Positive Behaviour Support (PBS) and anxiety-reduction techniques tailored to his preferences.
- A structured yet flexible routine to foster trust and autonomy.
- Weekly feedback meetings to empower him with control over his life.
- Collaboration with multidisciplinary teams, including psychological therapy focused on trauma, identity, and future planning.
- Compassionate support for his gender identity journey, creating a safe space for growth.

Emotional and Social Growth
Gradually, he began engaging with staff, memorising names, and forming meaningful attachments. He reconnected with his father, progressing from no contact to weekly phone calls and successful face-to-face meetings. In which he was able to have two successful face-to-face meeting that went well for both parties.

Mental Health and Behavioural Progress
His high-risk behaviours reduced significantly thanks to consistent staff practice and therapeutic interventions. His support needs decreased from 4:1 to 1:1, and he expressed wishes to return to his home town so that he can be closer to his family.

Health and Wellbeing
He adopted a healthier lifestyle, walking 10,000 to 12,000 steps daily and embracing balanced meals. His adventurous spirit led him to try new foods, especially seafood, fruits, and vegetables. His weight dropped from 80.7 kg to 75.6 kg, with a personal goal of reaching 70 kg.

Independence and Life Skills
He now manages his personal care, participates in cooking, does laundry with minimal support, and plans his own activities. His achievements include receiving a “You Can Do It” award for Functional Skills in English.

Celebrating Success
On October 2025, Act Care hosted a heartfelt farewell party to celebrate his incredible journey and achievements. Staff, therapists, and peers came together to honour his progress and bid him farewell as he prepared to return to his home town. The event was filled with joy, pride, and reflection, marking the end of one chapter and the beginning of another filled with hope and independence.

A New Chapter
The young person has successfully transitioned to a solo apartment placement in his home town, receiving 1:1 care. This marks a remarkable transformation from arriving at Enidaso House with intensive 4:1 support to living independently with dignity and confidence.
